Social Worker (Suicide Prevention)
Suicide Prevention Case Manager:
The SP case manager is assigned to the Mental Health & Behavioral Science (MH&BS) service line of VA NJ Health Care System.
The SP case manager will assist the VA NJ Suicide Prevention Coordinators to provide enhanced, intensive mental health follow-up care to veterans who are at high risk for suicide by tracking and monitoring their mental health treatment progress.
The SP case manager will maintain a collaborative relationship with the veterans, their family members/significant others (when applicable and appropriate) and other healthcare professionals with the goal of coordinating care and reducing the risk of suicide.
The SP case manager will on occasion assist with the provision of Suicide Prevention Training programs to both internal and external non-clinical individuals with the goal of heightening awareness of suicide prevention and reducing the risk of suicide among the veteran population.
